Key Responsibilities:
Provide screening, assessment and crisis intervention services via telehealth to adults referred by local police
Determine appropriate level of care and least restrictive setting for services
Provide transitional support, advocacy and care coordination in-person and/or via telehealth
Be available via pre-programmed tablets for crisis calls from local police
Respond immediately to crisis calls during scheduled on-call hours
Provide crisis intervention and ongoing mental health treatment to clients receiving intensive case management, including those on Assisted Outpatient Treatment orders or Not Guilty by Reason of Insanity
Deliver services in-person in office or community and/or via telehealth
Schedule and Shift Details:
Regular hours Monday-Thursday 8:30 am to 7 pm, Friday 8:30 am to 5 pm, Saturday 9 am to 1 pm; flexible scheduling available; some evening and weekend hours required; on-call responsibilities during business hours and voluntary after-hours on-call coverage
Travel:
Mileage reimbursement for personal vehicle use at IRS standard rate (.70 per mile)

Our client is seeking a Part-Time Registered Nurse (Home Health) to join their growing team in Schaumburg, IL. This role will be responsible for providing Home Health care to patients in the local area in the comfort of their own home!
Schedule and Shift Details:
Minimum – 16 hours per week (2) days per week
(8A to 5P) + assist in on call rotation:
(1) weekend day shift per month: 8A to 5P on Saturday or Sunday; will have scheduled visits ahead of time & seeing patients in the field – beginning 5pm, will be a true on call

Attend community meetings, conferences, and trainings and report to leadership

Collaborate with inter-departmental groups for program administration
Complete required documentation and reports
Act as liaison and participate in agency committees
Complete 24 hours of annual program-specific training
Work with Program Director and Finance to meet funding and budget guidelines
Support and train staff in Person Centered Planning guidelines
Ensure services and case records meet accreditation standards

Key Responsibilities
Perform Occupational Therapy assessments of patient functioning levels, plans short and long term goals toward improving/restoring independence and physical/cognitive functioning; and implement developed treatment programs, approved by the attending physician.
Provide guidance and instruction to patients/family members of developed therapeutic, creative, and self-care procedures/activities; and explain adaptive and progressive approaches to applying them.
Instruct the patient/family members in use of appropriate orthopedics, or prosthetic equipment; and may also design and fabricate customized adaptive and assistive devices, with training for their use.
Conduct functional and cognitive re-evaluations, as indicated, to assess the need for any revisions in the treatment plan.
Instruct and familiarize, as necessary, the patient and pertinent family members with all other aspects of the total treatment program.
Maintain adequate clinical and progress notes and submit all required paperwork in a timely manner according to Agency policy.
Periodically report to the physician on the patient's treatment, reponse, and progress, and assist the physician in determining any necessary changes in the treatment program.
Arrange for outpatient occupational therapy services that cannot be rendered in the home, as necessary.
Provide consultation to, and instruct other members of the home health team in the use of necessary equipment, techniques, modalities and basic activity programs, when needed, and as requested.
Prepare periodic reports, as required by the Agency which are sent to the physician and third party payers regarding care being provided.
Maintain and update professional competency through in-service attendance and similar professional resources.
Participate in in-service, case conferences, and staff meetings as required.
All other duties as assigned by supervisor.
